In the 1940s, the World Health Organization (WHO) defined health as a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity. Despite an industry upheaval and two gravely ill sons, this health-care entrepreneur discovered the prescription for perseverance. Sadly, looming business woes were the least of the Langdons worries that year. In the summer of 1996, at age three, their son Spencer–twin brother of Tyler–was diagnosed with leukemia, warranting three years of chemotherapy and approximately $1,000 worth of medication each month. Faced with circumstances that would obliterate the future goals of most, the Langdons didnt fold, but instead expanded ATPs services to override the Medicare massacre. Making ATP a onestop shop by adding outpatient rehab to its services was the ultimate goal, but first the Langdons had to move the company out of their Carrboro, North Carolina, home (situated in a bedroom, no less) and into a Hillsborough, North Carolina, office complex. To Ted, outpatient was ATPs salvation. I really started seeing the effects [of the Balanced Budget Act] in 1997, says Ted, 33. Medicare-based agencies were laying off staff, and the first people to go were with contract companies. If they didnt go Out of business or merge with other corporations, agencies drastically reduced their pay rates to contract companies, lowering the therapists wages. Some just didnt pay at all–like House Calls, a statewide home-health agency in North Carolina whose owner was nabbed for allegedly double-billing his way to Medicare/Medicaid fraud in 1997. I tried everything I could to retrieve [the $14,000 House Calls owed], but they were sued by everyone, says Ted. They had everything frozen, and I wa s so far down on the lawsuit list that, basically, I never got paid.

Real estate practice is a broad and widely defined area of the law. In a pristine sense it deals with the esoteric subject of real property law and titles. In practice the subject matter handled by real property lawyers is much broader. Many of the areas of law within real estate practice are areas of specialty themselves. Real estate lawyers examine and evaluate real property titles, handle real estate sales, purchases and other transactions, foreclose mortgages, represent developers and contractors in drafting and negotiating development and construction agreements, handle real estate financing, structure and draft the governing documents for planned developments such as condominiums, serve as counsel to community and condominium associations, draft and negotiate leases, litigate title, boundary, and other land disputes, and advise clients respecting zoning and land: use matters. The examination of real estate titles requires knowledge of corporations and other forms of business organization, wills, trusts and estates, and a practitioner handling transactions must understand contractual, business, and tax subjects as well. The subjects that are tested on the certification exam attest to this diversity. They are title issues; real estate finance; environmental and land use; condominiums and homeowner associations; landlord tenant; closings; sellers, buyers, brokers, and title insurers; real property litigation; taxation of entities and other tax issues; and ethics. Some hold an impression that at least some aspects of the real estate practice are not challenging, that the practice is increasingly title company, form, and paralegal driven, and that when business is light any lawyer with a good paralegal can Ado real estate closings. This impression is not accurate. As with any area of practice, successfully representing clients in real estate matters requires knowledge, skill, and experience. The diversity in real estate practice may mean that no one lawyer can be an expert in each practice area encompassing the modern real property practice or that are tested by the certification program. That, however, is not a reason to not seek certification. The committee recognizes that few lawyers are likely to possess specific and detailed expertise in each of the many subjects that fall within the scope of real estate.

John Osbornes first play, Look Back in Anger, had its premiere in 1956 and he called it a formal, old-fashioned play. His second play, The Entertainer, was set during the Suez Crisis and opened in 1957. What was new about both was that the characters were real working-class people. No real parts had been written about working- class characters before - only “cheeky chappy” roles such as Doolittle in Pygmalion. When asked if I wanted to go to see The Entertainer I was uncertain. Why? I had worked backstage on a brilliant amateur production (by the Tavistock Rep) in 1969 and it had stayed in my mind, with few productions since to affect the fond memories. How wrong I was to hesitate. For me this production, under Sean Holmess direction, ticks all the boxes and putting it on 50 years after it was set is brilliant timing. So much of it resonates in 2007: a misguided war in the Middle East and young men dying needlessly; the obsession with skinny/size zero women; fading performers (or “personalities”) trying to reinvent themselves; feelings against politicians of either party; comments about the Poles and the Irish; and confusion over national identity.

The new store, in Moorestown, is doing some interesting things with skin care.Open-mindedness toward innovation in skin care merchandising is not limited to the chain drug channel. Supermarket retailers, long the also-rans among mass marketers in the category, are beginning to experiment with larger skin care departments and higher-end products.One regional Sunbelt chain plans to increase its skin care mix by a third before the end of this year, with higher-priced facial products it had not carried before accounting for almost all the increase. The retailer also plans to experiment with free-standing skin care displays, end-caps and promotional price reductions this winter. It will double the amount of space devoted to the category in most of its stores and begin running ads devoted solely to skin care for the first time.
Forty-two percent of all respondents agreed that supermarkets “need less health and beauty and more unique groceries.” (25% agreed strongly.)

From a public perspective, what is the core identity or role of the chiropractic profession in health care? Are chiropractors mainstream or alternative, primary care providers or specialists, and what is the best brand or identity for the profession?Since May, the WFC has been collecting e-mail databases from its 80 member national associations and others. All e-mail addresses have been stripped of names and other information in order to protect privacy rights. This gives the WFC information for every chiropractor who has an e-mail address and has furnished it to his or her association. I have known this beach ever since I can remember, and my father also knew it before me. When I came here as a child the little shop and the caravan park did not exist, and access was via a long twisting track that led off the main road. We came for holidays three and sometimes four times a year. I was taught to swim in the creek, and taught how to fish on the beach, and how to get sandworms, yabbies and pippies, which we used for our bait. My father knew the beach very well, and with the passing of time ! came to know it even better. As the old fishermen say, I was taught to read the beach, to spot the gutters for the whiting and flathead, to cast well out with a long beach rod when the tailor were running, to catch the blackfish in some of the deep holes in the creek, or when all else failed, to go after the large mudcrabs that lived around the mangroves. The supply of seafood was never a problem, but we only took as much as we needed. Any waste would make my father angry. I was never allowed to swim in the surf. This was accepted without question from ever since I can remember. Even in later years when in my teens, and when sometimes a school friend came on holidays with us, it was on the understanding that we only swam in the creek. Provided we kept well clear of the oyster-covered rocks on part of the northern bank there was no problem, and the dogs and I spent a great deal of our time in the water there, or the dogs waited patiently, digging themselves into the cooler part of the sand while I chased worms or yabbies at low water. We always came to the beach when the tailor were in season, and we always seemed to be there when the big sea mullet were on the boil and were caught with weighted jags and a well-aimed cast. Today with the number of fishermen that descend on the beach and the number of dangerous jags that fly through the air, this pastime has become as much of a hazard to the fisherman as it is to the mullet. Also, there are now always commercial fishermen with nets, lurking beyond the breakers.

“There are [more than] 30 million Americans who have no health insurance and another 20 million who have inadequate health insurance,” says Rep. Louis Stokes (D0Ohio), chairman of the Congressional Black Caucus Health Brain Trust. “It is mandatory that we begin to try and give Americans–particularly minorities–adequate health care as a right.” The Bush plan would give people not covered by Medicaid tax credits, and families making less than $80,000 a year tax deductions to pay for medical costs. He would also pass laws to ensure that even chronically ill patients could receive health insurance. The plan has received a cool reception on Capitol Hill, and alternate proposals, including a full-scale, government-paid national health insurance plan, are under development. But what does America really want for its health care system? The views are many.
The American Journal of Obstetrics and Gynecology 2002;187(3), Part 2, focuses on how and why womens health and gender-based competencies should be woven into the fabric of undergraduate medical education. It discusses appropriate components of the health and medical curriculum, and combining health care competencies with educational objectives and models for teaching reproductive health.
Part II, explanatory economics, represents the bulk of the book. The economics of demand, price elasticity, risk aversion, and the effect of insurance on the demand for medical services are explained clearly and well. Behavior of suppliers in the medical care market is reviewed, with attention given to differences in for-profit and not-for-profit organizations. A solid explanation of competitive markets allows discussion of timely issues such as the limits of applicability of the competitive model to the medical care market, price and nonprice competition, and market power of providers, purchasers, and patients. Outside readings on current managed competition proposals would augment the textbook discussion well.A short section on the economic effects of information asymmetries in the medical care and health insurance market introduces the concepts of supplier induced demand and adverse selection. Both discussions are short, but the portion on adverse selection especially needs expansion. More rigor, explanatory graphs, and a more extensive bibliography would make the adverse selection concept more useful. This topic is critical to a sound understanding of the functioning of medical expense insurance.The final section deals with value judgments and evaluative economics. The nontechnical explanation of efficiency versus fairness criteria for evaluating medical care policy is somewhat cursory. More technical material is needed to improve the precision of this discussion. However, even without any additions, the text provides a general background on issues of access to medical care, affordability, and quality of care. The difficulty of making allocation decisions given scarce resources is clearly laid out and opens the way for a discussion of policy alternatives.

Laminate flooring remains one of the fastest-growing applications for composite panels. Laminate flooring has been the wunderkind of flooring products since it was introduced to the United States marketplace in the 1990s. Since then, several new factories have sprung up in North America dedicated to the production of laminate flooring planks, and in some cases, tiles. The death rate due to colon cancer in the United States is second only to lung cancer, claiming the lives of nearly 50,000 people annually. Both men and women contract the disease in equal numbers, and die at the same rate. The risk for colon cancer begins to increase after age 40 and rises sharply from 50 to 55. The good news is its one of the most preventable cancers. “With what we know about colon cancer, we can take significant steps toward preventing one of the most common and deadly cancers today.” says Moshe Shike, M.D., director of the Memorial Sloan-Kettering Cancer Prevention and Wellness program.
Intestinal polyps, mucosal protuberances from the epithelial lining of the lumen of the colon and rectum, can vary in size and shape, occur anywhere in the colon or rectum, and vary in histopathology. They usually do not cause symptoms unless they are large enough to cause obstruction or are vascular and bleed. By gross appearance they are classified as sessile, semisessile, flat, or pedunculated (Plates 1–4), and can be either neoplastic or nonneoplastic. The result of histologic studies is the only final determinant of the nature of a polyp; the gross appearance or the size and shape of the polyp is of limited value in terms of management, treatment, and prognosis. The greatest concern with polyps is their potential to become malignant. Studies have shown that early identification and removal of polyps can reduce the subsequent occurrence of invasive cancer. Studies have also shown that most colon cancers have developed within previously benign adenomas.[1-6] Therefore, currently accepted management is to remove all colonic adenomas.[7-14] The epidemiology of colonic polyps indicates that prevalence is higher in North America and Europe than in other parts of the world. According to autopsy surveys in the United States, nearly one half of the population harbors at least one adenomatous colonic polyp.[7] Among persons over age 65 years, more than two thirds of the population have colonic adenomas.(7) The prevalence of adenomatous polyps increases with age. Polyps of larger size with a higher degree of dysplasia as well as multiple adenomas are a common occurrence in persons over 65 years of age.[7] Multiple polyps are found in 28% of patients who have at least one adenoma. One third of all surgical specimens containing colon cancer also contain an adenoma.[7]

Budget cuts are seriously threatening the dental health of adults and children with special needs, says a report published today. VITAMIN D could improve fertility in some women, a study has found. The researchers, led by a group from Yale University School of Medicine, said those with ovulation problems or PCOS were far more likely to be lacking in vitamin D than the other women. Vitamin D, found in oily fish and eggs, helped to restore regular periods, thereby boosting the chances of conception.
A total of 91 women, aged 30-69 years, participated in the study; 50 were nonsmokers and 41 were smokers. The subjects completed questionnaires on smoking status and dietary habits and provided blood samples for nutrient analysis. Serum levels of [alpha]-carotene, [beta]-carotene, cryptoxanthin, and lycopene in smokers averaged only 71-79% of the concentrations found in nonsmokers. Dietary differences could not account for this effect. Mean serum concentrations of vitamins C and E and lutein/zeaxanthin were only slightly, and not significantly, lower among smokers, and concentrations of vitamin A were actually higher. The results for carotenoids are generally consistent with those of other studies. The lack of an association between smoking and serum vitamin C levels may reflect the relatively high vitamin C intake of this study population. The authors observe that the negative effect of smoking on serum concentrations of antioxidant carotenoids may pose a serious health risk in low-income populations already at higher risk for many chronic diseases.

SMALLBUSINESSAMNESTY The small business amnesty, as announced by the Minister of Finance in the 2006 Budget Speech, runs from 1 August 2006 to 31 May 2007. Details of the amnesty are available on SAICAs Tax Page. SAICAs National Tax Committee has identified small business tax as a strategic research area and established a task team to review and make recommendations on the current tax policy regarding small business and the tax compliance burden facing small business.
Who was present at the creation of certification of legal specialties in Florida? Tax lawyers, of course. Theyre everywhere. Nothing is as certain as death, taxes and more taxes. Another major contributor to the tax certification program whose work should be recognized and appreciated is Hank Raattama, who was an original member of the committee which drafted and implemented the tax certification plan and has remained a member to this day, with 21 years of service. The purpose of the program is to assist the public in making an informed decision when seeking and selecting an attorney with special knowledge, skills, and proficiency in the area of tax law. The Supreme Court of Florida approved the board certification program in 1982. While all lawyers are allowed to advertise, only certified attorneys can identify themselves as Florida Bar Board Certified or as a specialist. There are currently 281 attorneys board certified in tax law, which is about seven percent of all board certified attorneys.

Britney Spears left a live-in drug rehabilitation facility early Wednesday in Malibu, Calif, less than a day after checking in, according to several reports. The TV show “Extra” and the celebrity Web site TMZ.com said the 25-year-old pop star had left the center far short of the usual 45- day stay. TMZ said she couldnt handle rehab and went home.
George Carlin is entering a drug rehabilitation facility to shake his dependence on wine and a painkiller. The name and location of the facility werent disclosed.

Editors Synopsis: The author examines how changes to the Uniform Commercial Code Article 9 relating to promissory notes secured by real estate mortgages may affect priority disputes between secured parties and the Internal Revenue Service. This analysis proceeds upon the following basic assumptions: First, the holder of a note and mortgage may transfer that holders rights in the instrument by a document of assignment without a physical delivery of either the note or mortgage. This concept is implicit in the Article 9 provision for automatic perfection of an absolute assignment of a promissory note.6 Otherwise, because possession is also a method of perfection,7 the provision would be superfluous. Second, attachment (i.e., creation) of a security interest8 in Article 9 collateral, including a promissory note, requires either (1) a security agreement authenticated by the debtor, or (2) possession of the collateral by the secured party pursuant to an agreement.9

Petersen Automotive Museum presented the event, which featured vehicles including a 1925 Rolls-Royce Phantom I Aerodynamic Coupe, a 1939 Bugatti Type 57C, as well as automobiles from Bentley, Mercedes, Packard, Duesenberg and other rare and valuable makes.

This film is visually entrancing (the sequences set at the track are sufficiently stimulating, chaotic and noisy with furious angle cuts to engage your basic attention-deficient kid) and often slyly comic (because there are only cars in this world, tractors do the work of farm animals and one popular prank is tipping over tractors) and generously garnished with characters that can be personable (the wonderfully naive beat-up pick up voiced by Larry the Cable Guy is as endearing and appealing as any Disney character since Thumper). But it lectures, often heavy handedly, about a lost America, dated here to the mid-1950s, a world of caring neighbors, a slow down, small-town world of simple pleasures like a slow drive in the country, where love was expressed in batted eyes and stammering speech. Granted that 1950s is a confection of Hollywood and perhaps it is an exercise in pointless nostalgia to mourn something that never really existed, it works here. When Lightning McQueen, the showboat hot-shot race car (Owen Wilson), loses a big race (his daredevil antics on the track cost him the victory) and decides brashly to eschew his racing team to head alone to the season-ending showdown race for the Piston Cup, he gets lost on a stretch of back highway, the mythic Route 66, in the deserts somewhere on the way to California. He ends up in a lonesome town, Radiator Springs, itself a cross of intersections bypassed long ago by the interstate and now settling into dust. When Lightning ends up impounded in the town and then sentenced by an unforgiving old-timer judge (Paul Newman) to community service–he must repave the stretch of broken-down highway his race car has damaged–the young kid comes to understand, slowly, the simple values that his media-driven celebrity lifestyle and virtually impenetrable narcissism have never shown him: friendship, trust, honesty. Happily, the judge turns out to be himself a relic of past racing glory and with his help and his patient (although wonderfully gruff) instruction the kid learns a thing or two about handling cars. When Lightning returns to race, he does so richly educated in the importance of tradition, the value of modesty, and the sterling quality of friendship. I would not dream of spoiling the end of the film, but in the end we learn, as with all truly significant sports films, what happens in competition pales next to the significance of what happens off field. With Lightnings newfound sensitivity, the town finds its way to a renaissance that is as believable as it is heartwarming.
